When you advance to checkout, you will be prompted for information for the To/From sections for the gift card. We will pre-fill the To/From sections based on your responses to the form, and in the absence of responses to the form, we will pre-fill with the shipping and billing address names, or leave the lines blank in the case of identical shipping and billing address names.
